Care planning enables information to be gathered, taking in to consideration an individual’s biological, psychological, sociocultural, environmental and politico economic status. These factors are incorporated in to the care planning process to enable an individualised care plan that meets the holistic needs of the individual (Doenges and Moorhouse, 2012). The aim of this assignment is to explain and explore an individual approach to care planning and how using a nursing process and nursing model collectively will provide a holistic approach to care.
California Supreme Court Clarifies Long Term Care Act’s Application to Release of Confidential Information The California Supreme Court has clarified the application of the Long-Term Care Act’s disclosure requirements in consideration of Welfare and Institutions Code section 5328’s general prohibition against the release of information contained in the course of providing treatment to mentally ill and developmentally disabled individuals. In State Dept. of Public Health v. Superior Court (2015) 60 Cal.4th 940, the Supreme Court considered the issue of whether the disclosure requirements of the Long-Term Care Act (LTCA) or Welfare and Institutions Code section 5328 applied where a public records request was made for health records. The case involved the Center for Investigative Reporting, a news organization investigating the treatment of mentally ill and developmentally disabled in state owned health care facilities, which issued a public records request to the Department of Public Health (DPH) for copies of all citations issued to the facilities it was investigating.
I would like to commend Judy Levin for her work in advocating for LTC. Judy has been promoting LTC issues for at least 10 years. As an advocate she has been a quiet warrior, a good organizer, and an indomitable force for change. I became aware of Judy more than 10 years ago, while she was advocating in the DC Office of Aging. I did not see her for nearly 9 years, but heard her name mentioned frequently.

Hello, Mary. The goal of beneficence is to protect the well-being of the patients. In nursing, doing good and doing the right thing is the best policy. In our long-term care restraints is not an alternative. Instead of using restraints our facility provides patient care observers to be with the patient to ensure their safety.
However, these different lifestyles for elderly people has so much variation to it that there is a plan for almost everyone. The only thing stopping people from having the perfect plan is the cost that is usually associated with these plans because the severity of each elderly varies heavily. For example, my great-grandfather, who is in his late 90’s, is still able to have a coherent conversation with my family and knows exactly what is going on in the world. As a result, he lives in assisted living, where he can a life how he wants to, without a heavy regiment. I don’t think there is a perfect solution in taking care of elderly people because you either give them the best care or you become financially broke of taking care of an elderly person.
As well the program's mains goal is based around the desire of the elderly to remain in the comforts of their homes, keep their independence, and ties to the community. Such as attending church, family functions, and leisure activities. The program also addresses their desire to live out their later years in the care of their loved ones. Furthermore, the proposal takes into consideration the desire that the family member has to take care of their loved ones. However, we understand that family member risk poverty themselves by doing so.
Different people may criticize adoption of the system but their points have weak foundations. From different perspectives, such arguments tend to support the inefficiency that is persistent in most healthcare facilities. Application of the systems is seen to take of everyone’s welfare while improving the economy of the country. Moreover, success in other developed countries shows that the system is not difficult to apply. The government also needs to consider issues such as viewing of healthcare access by individuals as a right.
As the number of older Canadians increases, there will be a major financial and labour strain on the Canadian public health care system which will require increased government funding and social programs geared towards elder care. There are a number of areas of care that elderly citizens require that are not fully covered by the public health care system. As the population ages their naturally declining health increases their need for such things as prescription drugs, home care, long-term care and end-of-life care. With such a large number of the population expected to require these services over the coming decades, new programs will need to be developed and funded to meet these needs. The strain on the labour force will be seen in two ways.

The long - term nursing facility has contracted with the community hospital to provide a program for patients recovering from heart failure. The goal of this program is to provide care and prevent hospital readmisstions for patients recovering from heart failure who cannot care for themselves in the home setting. Current chart audits on nursing care provided to patients with any type of cardiac related illness reveal that the nursing staff is unable to recognize a change in condition, especially in regard to physical signs of fluid retention in the lungs, abdomen, and lower extremities. Whether government is the party which should hold the most responsibility in the case of employment and social security policy has always been a debating issue. By looking at the effectiveness of the recent policies that gathered the effort of government and employers, we could examine that whether each party is doing their exact part in contributing to solve this problem, which will eventually “address Britain’s supposedly broken society and restore economic competitiveness” (Jay Wiggan, 2012). It has been unable to respond to our evolving job market and changing nature of our workforce. Society has changed but the benefits system has failed to change with it. So it is time to bring welfare into the 21st Century.
