MTM is used to describe the broad range of health care services provided by pharmacists. These services include comprehensive medication reviews, medication reconciliation, drug use review, the ordering and review of lab tests, immunizations, drug dosage adjustments, and identification of gaps in care. Integrated systems of care, such as accountable care organizations (ACOs), already view MTM as essential to care delivery and to meeting ACO quality and cost targets. Such organizations also are heavily invested in HIT, including e-prescribing and EHRs. MTM can improve medication adherence and patient outcomes among patients suffering from chronic diseases, thus cutting costs and improving the quality of care and patient
More importantly, I become fascinated with the field. Pharmacy appeals to me as an intriguing and promising occupation. More importantly, it is also a noble profession. I’m currently volunteering at Massachusetts General Hospital. I started off volunteering in the department of patient escort services and then I was transferred into the emergency room.
Collectively, we urge your support for S3111 and respectfully ask for the bill to be posted for a vote in the Senate Budget Committee. S3111 was passed out of the Senate Commerce Committee with unanimous support. We urge the members of the Budget Committee to lend their support for successful passage of S3111.
Secondly, the way the resident receives his medications should consist of the CM stating what each of the medications are so the resident is aware what he is taking. By implementing this, the CM can do the final check of administering the medications. If the medications themselves could be barcoded and scanned in before popping the medication in the medication cup, this would help the CM double check the five rights as well. A bar-code electronic medical administration record (eMAR) technology associates several technologies into the medication administration process to provide the correct medication, dose, time, route, and patient. This technology will provide an additional check and implement safety (Poon et al., 2010).
As a hospital pharmacist you are expected to be an expert in the field of medicines, how they are used and their various effects on the human body. As hospital pharmacist you will be responsible for dispensing prescriptions, purchasing medications and quality testing of medicines. Hospital pharmacists may also manufacture medicines, as in some extreme cases a medicine will need to be tailor made for an individual patient. B. Advantages: Clinical aspects
Medication Errors in Healthcare The nursing profession entails many responsibilities that range from providing emotional support to administering medications that could result in death for those receiving care. Approximately 40% of a nurse's day consists of passing medication, a duty that sets their level of liability above many other healthcare professions (McCuistion, Vuljoin-DiMaggio, Winton, Yeager, & Kee, 2018). Despite today's advances in technology and nursing education, the frequency of medication errors is still staggering. To ensure that the benefits of nursing outweigh the risks, nurses look to the Quality and Safety Education for Nurses (QSEN) six core competencies for guidance.
Because of this, I didn’t have to spend a lot of time looking up different drugs and what they did. My second objective was to learn everyone’s names right away to create a more open environment for me to ask questions and learn as much as possible. This made it much easier to approach different staff members with a question or concern. My final objective was to ask pharmacists or techs each day if they needed help with a project. This objective didn’t apply as much since our days were more planned out than I had expected, but I did manage to frequently ask about things I could do to help and things I could work on to learn the most that I could.
Safe medication administration is a big aspect of nursing care, because if medications aren’t given safely, then it can lead to some serious adverse effects to the patients. There are many things that can go wrong, and that’s why nurses have to be very careful when handling and giving medications. Nurses can make mistakes, and give the wrong med, give it to the wrong person, or even give too much or too little of the drug. Careful medication administration can lead to not making big mistakes that can lead to hurting others. “Medication Administration is a complex multistep process that encompasses prescribing, transcribing, dispensing, and administering drugs and monitoring patient response.”
The number of acquisitions is not so high and it depends more upon the organic expansion. • These pharmacies face informative and predictable risk from the variability of “generic conversion” • Inability of the company to keep stride with the growing private labeled brands popularity. • The in-store implementation of the store formats and services is not consistent at every
A pharmacist also has a key role. Pharmacists are healthcare providers that have to communicate with there patients to make sure they can take the medication properly according to their lifestyle, diet, and transportation. The best reason of all is that I get to work directly with patients. Pharmacists are able to see patients anytime during the week, morning, evening, and
I have been lucky and honored to work with an amazing team of pharmacists who were always there to answer any of my never ending questions. One aspect that made me fall in love with a pharmacy career was the special interaction between pharmacists and patients. Another reason I have decided to pursue Doctor of Pharmacy degree was the fact that I have seen many refugee patients who were unable to understand why they were on medications. One day, I assisted a new patient to the pharmacy who did not speak English but luckily, I was able to interpret in Arabic language. She needed to get her medication refilled, when I asked her which one, she replied "the one I take for fat".
Pharmaceutical Care Patch Adams is a 1998 semi-biographical comedy-drama film based on the life story of Dr. Hunter "Patch" Adams and his book, Gesundheit: Good Health is a Laughing Matter, by Adams and Maureen Mylander. (Wikipedia) The movie is all about a medical school student, Patch Adams who is eager and passionate in helping patients in a way which his dean disagreed on. Despite being warned by his dean and lecturers, he still holds on his principle in treating the patient as a person, not treating the disease.
Pharmacists oversee everyone in the pharmacy and therefore must be able to communicate with their subordinates appropriately and efficiently (“What Pharmacists Do”). It is their job to facilitate a good relationship between all their coworkers in order for the pharmacy to run smoothly and resolve any issues that may come up (Christakis 1,3). Pharmacists interactions through interpersonal communication keeps the workplace running smoothly. My overall academic and working experiences in the past have prepared me to be an

How do clinical pharmacists care for patients? Clinical pharmacists: • Provide a consistent process of patient care that ensures the appropriateness, effectiveness, and safety of the patient’s medication use. • Consult with the patient’s physician(s) and other health care provider(s) to develop and implement a medication plan that can meet the overall goals of patient care established by the health care team. • Apply specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, in performing their patient care activities in collaboration with other members of the health care team.
