Alejandro is a 30 month (2 ½-year-old) male of Hispanic descent. Currently, he is a ward of the Los Angeles County’s foster care program (DCFS). He has been placed in a private foster family agency’s (FFA) volunteer foster family home. His mother, Ms. C., is a 28-year-old woman who is currently living at a residential drug program for treatment of her addiction to methamphetamine. Ms. C. self-identifies as Catholic. Alejandro’s father, reportedly, has not dealt with Alejandro since after his birth. When Alejandro was approximately 12 months (1-year) old, DCFS removed Alejandro from Ms. C. due to severe neglect. Currently, Ms. C. is attending court mandated parenting classes, a drug rehabilitation program, and bi-weekly visitations with her son.

I am employed as an on-going social worker by the MA Department of Children and Families (DCF). DCF is a child protection agency, which is responsible for protecting children from abuse and neglect and strengthening families. The Department has offices throughout the Commonwealth; I am located at the New Bedford area office. As an on-going social worker I am assigned families after a report of abuse or neglect has been reported, investigated and supported. It is the on-going social worker’s responsibility to provide professional child welfare social work services, through home visits, to the family. Since the beginning of time in the United States, the idea of the American Dream has had a heavy influence on society. According to Document C, American Dream is defined as earning enough money to be happy, obtaining a worry-free lifestyle, and running in a high social circle. While working Americans still hold on to the hope of the American Dream, individuals from other countries often move to the United States with the hope that they too may have a piece of the pie. Despite the novelty and allure of the American Dream, it is nothing more than false hope. The American Dream is unachievable in the United States because no matter how hard one works or how hard one tries to save money, the American Dream is simply not accessible to those that begin with nothing.

i.10The sponsor is providing the minor with a safe environment. The placement is located in an apartment complex in Woodbridge, VA. The placement appears to be safe and no visible hazards were observed. The minor is provided her own bed and shares the bedroom with other sisters. The minor did not report any concerns regarding her safety, the placement or the community. The minor is aware she may report any concerns to the CMS, sponsor, teachers or other authorities. The sponsor will continue to provide the minor with a safe environment.

Furthermore, including the family when treating a patient generally holds the best results. At the same time family does not have to mean blood relative, in reality family is whoever that individual deems their family their support system. The CP addressed with Mrs. Navarro about the CP conversation with Dr. Freddie Marton with regards to Tyler’s medication regimen. The CP asked Mrs. Navarro if she is providing Tyler’s with his medication and her response was that she needs to do a refill and then she says, “Oh, I cannot find them since we are packing.” However, Tyler intervenes and stated that he knows where the medication is and based on Mrs. Navarro reaction, she appeared stunned. Tyler left the living room and returned with the bottle at which appears to be full. The CP glazed at Mrs. Navarro reaction and she immediately stated that she will start giving him his medication and will also considered the school nurse providing Tyler with his medication at the school. On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment.

On 7/9/15 worker made an unannounced visit to the residence of Ms. Bernice Connell, for the purpose of making first victim contact. Ms. Kayley McKinnon, granddaughter-in-law of Ms. Connell greeted worker at the door and showed worker to Ms. Connell 's room. The room was cluttered but did not present with an odor. Ms. Connell was lying in bed watching TV, she was appropriately dressed with good personal hygiene.

Once Hazel entered the program, she begin working with DHR and Secure Dwellings to develop a plan for her to get her child back. Hazel’s son is now back with her and his youngest brother, a family reunited because of the stability the program afford her. We are currently assisting Hazel and DHR with getting Hazel permanent housing and employment in Birmingham.

Resident E.V. has demonstrated the ability to cope by displaying effective coping patterns. She has the desire to become both healthier and live a healthier life style. Her goal is to gain recovery so she can walk on her own again and eventually move back into her house to help take care of her husband, whom has dementia. She is willing to modify her lifestyle accordingly if she is unable to gain complete recovery. However, she would like to be as independent as possible. Although she is used to being in control of everything, resident E.V. has accepted help from others including her family, neighbors and friends in order to take care of her husband. Her family is adapting to her being in a long-term facility and has taken over in helping take

The reporting party (RP) stated there were a list of issues and concerns regarding the level 4 home. A resident named Tammy Sanchez was observed by Regional Center counselor staff member Constance Sifton standing on a street corner on Mooney Blvd., in front of MOR Furniture store unsupervised. The counselor from the Regional Center pulled over and spoke with the resident. The resident disclosed that she had been on the street for an extended period which was indicated by the resident being drenched in sweat and appearing dehydrated and shaking. The resident was initially with a staff person named Isabelle and her boyfriend. The resident disclosed that the caregiver and her boyfriend went inside a building and she was instructed to remain in the boyfriend 's truck.

Ms. Stevens and her child, Tjayda Stevens are receiving preventive services with Harlem Dowling Westside Center for Children and Family Services (HDWC). Ms. Stevens’ case was referred from ACS and accepted thru HDWC on 03/02/2015. The services provided to Ms. Stevens are casework counseling, community advocacy, housing advocacy, and monthly home based visits to ensure stable family functioning and child safety. Ms. Stevens has been receptive to services since the agency obtained case planning responsibility. Administration for Children’s Services recommended the following service plan for Ms. Stevens to complete parenting and anger management class, individual counseling, and domestic violence.

One day she received an email from her old friend, Mot, that she had met while working at the shelter. He was a veteran who is homeless and was suffering from some mental health issues. Mot had been homeless for quite some time. He loved his life as a drifter you might say. He was quite accomplished at getting things done for himself as long as he was near a Walmart and a Library.
