In order to realize this goal, the VBA established, as one of its goals, the necessity to “Empower employees to serve Veterans reliably, efficiently and with compassion”. Nested under this goal, VBA identified the objective to “Optimize processing capacity to meet the forecasted demand for services”. This initiative will focus on adopting descriptive identifiers for the VBA workload, which clearly indicates the type of work performed. Additionally, this initiative will provide greater definition and clarity to the system of broad categories, which is currently in place.
Strategies In the past there have been arguments between agencies and professionals over funding and arguments over who does what, which obstructed closer professional working. A number of strategies have now been developed that focus on improving co-operation for the benefit of those using services. 1) Multi-agency working: The support planning process and single assessment process have inspired bigger inter-agency cooperation with the individuals needs being central to the process.

His book, “When Healthcare Hurts” explains and answers the ways serving can affect those who need assistance if not trained appropriately. According to Greg Seager, “Right now many short term and long term medical teams may be doing more harm than good”(Seager, pg.xv.). The purpose in this book is to educate the reader is to talk through the best practices by giving advice and information of the essential ways to prepare the missionaries. Reading the book the reader can notice how the book's main points are structured with practice guidelines, knowledgement of mission, and being self aware.

The case worker along with the client, payee, guardian, or other responsible parties collectedly create the service plan. The case worker present the ideal, but it is the client’s job to complete all paperwork, applications, or request outside services. Sometimes creating that balance of self –determination and vulnerable is often times a challenge, because the values and belief of a client can be totally different from a social worker. Even though case workers feel strongly about a situation it is the clients right to do as their please. For example, I and Level 5 caseworker were doing visit one day.
In this model medical and non medical professional staffs are co-ordinate by a case/care manager to address the needs of a client. Case meetings, care planning and exchange of information are coordinated by case manager. An individual care plan is often the product of case management meetings. For example, “For over a decade, the Defense Department’s Safe Helpline has provided a secure place for military survivors of sexual assault and their loved ones to get support and information 24/7.” (Vergun) . In addition, the helpline safe is properly trained to provide specific support to survivors in any of the military task forces and can also help connect service members to resources within their communities with ease. Another program that the organization had created to help support safety amongst their employees and civilians is by implementing a program called the “Patient Safety Program” . This mission of this program is not “ promote a proper culture of safety to end preventable patient harms but to also engage, educate and equip patient care teams to put evidence based leading practices in place across the organization.

SSGT Joseph White came into the local VA Community based clinic for medical care. When he was discussing his situation at home, the medical doctor found that it would beneficial that the social worker talk with SSGT White. The social worker obtained an appropriate assessment to assist with finding services that would be available to him through the VA and/or local community organizations for his symptoms and treatment options. When establishing a relationship with a potential client, building and keeping a rapport with the individual is key to a positive working relationship. The individual needs to feel that they are respected, understood, and listened to when the relationship begins.

The Chronic Care Model allows improvement of patient safety, effective care, patient centered, timely care, efficient care and equitable care. This will decrease cost waste, which will lead to a better outcome for the patient. The model allows a patient and a specific group of people to work together to improve outcomes. It also allows the group to monitor the patient’s performance over time.
