Health Impact Assessment-Provides advice to communities on how they can stay healthy- by evaluating a potential plan, project or policy before it is “built or implemented”(CDC.2015). For example: Maryland state code requires fire sprinklers to be installed in new and renovated single-family homes. Statistics show an 80% decrease in the chance of death in homes equipped with sprinklers. This regulation would also be considered Health in All Policies(HiAP) due to its cross sectoral nature.

Provide an example of how a “health coach” can help bridge the gap in the coordination of care for a patient with complex medical problems, and two reasons stated in the article why a patient may be more receptive to a health coach vs. a healthcare provider. A “health coach” can help bridge the gap in the coordination of care for a patient with complex medical problems in many ways. These health coaches can provide knowledge and support for patients who do not know how to properly manage their diseases/medical conditions. These coaches can also encourage patients to cut out bad habits (ex. Smoking) that cause their medical conditions to escalate. Health coaches can be extremely helpful in providing preventative care for patients that need guidance and assistance.
Studies have shown that peer support leads to improvement in self-management behaviors of dietary modifications, blood glucose monitoring, and increased physical activity (Murrock, Higgins, & Killion , 2009). Social support provided by peer, family and community members have been effective way to motivated individuals in continuing their attendance in health programs. Health behavior change theories and chronic disease management models highlighted that family support influence individuals affected with chronic illnesses into engaging in self-care behavior (Warren-Findlow & Prohaska, 2008). They are more encouraged by family members to engage in physical activity or weight loss programs. Peer support is also effective among individuals coping
A patient 's motivation to adhere to prescribed treatment by increasing the perceived importance of adherence, and strengthening confidence by building self-management skills, are behavioral treatment targets that must be addressed concurrently with biomedical ones if overall adherence is to be improving (Mathevula, 2013a and Mathevula, 2013b). c. Health care team/ health system related factors: Relatively little research has been conducted on the effects of the health care team and system-related factors on adherence. Whereas a good patient-provider relationship may improve adherence, there are many factors that have a negative effect. These include, poorly developed health services with inadequate or non-existent reimbursement by health insurance plans, poor medication distribution systems, lack of knowledge and training for health care providers on managing chronic diseases, overworked health care providers (Stroke Association.
I strongly value my health and believe that health is our greatest form of wealth. When one lives a healthy lifestyle, it means more opportunities to explore the world, build families, achieving anything the heart desires and conquering personal goals. The behavior change philosophy fits my personal philosophy of health education the most because it involves goal setting, behavioral contracts and self-monitoring to help foster the modification of an unhealthy habit. The behavior change philosophy is very important because change is a process, not an event. Self-efficacy and motivation are key factors in successful behavior changes.
Wellness in chronic illness is the topic for this discussion board. A chronic illness can potentially cause limitations in quality of life, productivity, functionality status, and worsening overall health ("Living well with chronic illness," 2012). Despite these negative aspects a chronic illness can bring, I believe that a chronic illness does not need to define an individual, and that wellness with a chronic condition are not to be considered an oxymoron. Self-management of chronic illness can impact the wellness of an individual. Teaching the individual to manage, treat, and identify problems with the illness that may arise gives responsibility to the individual that is affected (Gready & Gough, 2014).
Introduction People hope and seeks long and healthier lives. Thus, health care is the act of taking preventative or necessary medical procedures to improve people well-being. Improvement or preventative may be done with surgery, the administering of medicine, or other alterations in a person 's lifestyle. These services are usually offered through a health care system made up of hospitals and physicians. Although, the health care system is set up to reduce or to prevent disease etc., there is a gap or disparity in the US health care system.
Patient empowerment is also considered to be an approach to health promotion involving patient’s autonomy. Ensuring patient is actively involved in their care, allows the patient’s to achievement of self-efficacy of having a sense of control in their own life. Studies had shown that, by empowering the patients in decision making, there was a change in behaviour and life style. Patients took control of their own health and wellness, which lead to decrease in dependence on healthcare service. Patient’s previous experiences had influenced their level of empowerment in decision making.
whenever patients encounter frequent visits to health care professionals (Beck et.al, 2002). Most often patients follow the treatment plan and carry out behavior change such as exercising or modifications in diets when they are informed about their health condition and are involved in decision making (Beck et al., 2002). This is true, simply because people tend to follow perceptions and decisions made by their own or with the help of professionals. For instance, one, probably, would not reduce alcohol consumption if he/she will not understand to what extent the human body can be damaged if this habit left unattended. When a patient realizes what are the potential negative outcomes he/she may face with, it would not be problematic for him/her to set priorities.
The goal is to add to the quality of life for an individual improving their
In relation to caring for this patient I displayed strengths in implementing appropriate nursing actions, adapting to change and communicating effectively with the patient to provide patient centered care; however, I discovered weakness such as, a lack of confidence and patient teachings. I believe that the nursing actions I created for my patient this week were well thought out and my ability to adapt to my patient’s changes and mood to implement my actions greatly improved my patient’s overall health. Comparatively, this patient was my first patient I administered medications to and it was evident that I was nervous when explaining the medication’s purpose and actions. However, since then, with practice and my cheat sheet I feel more confident
Chronic illnesses are serious diseases which could last the whole life of the concerned person, affecting the person’s mental, emotional and social wellbeing (Corbin and Strauss, 2013). In order to understand health and illness and the impact of health several assumptions have been. Being privileged to care for a client with type 2 diabetes mellitus, I was able to observe that though my clients A1 c which was very high at the initial stages, gradually it reduced with the management plan .The deteriorating signs and symptoms which seemed to be progressing rapidly declined. This supports the assumption that the course of chronic condition varies and changes over time.
Currently, there seems to be a large trend towards healthier eating and healthier living and have therefore led to a boom in smoothie businesses across the country. Even with this boom, there are many areas where smoothie bars are lacking. Given the market availability and the potential profitability a smoothie bar can offer, now is the perfect time for me to make my dreams of having my own smoothie company, No Fuss Smoothies, a reality. My target audience is the middle and upper class health conscious individual age 36 up.
