Introduction Nursing judgment refers to a clinical assessment concerning person’s response to health situations or how vulnerable the response is to individuals, household, clusters or the entire community. Clinical judgment consists of two main sections, descriptor, and attention on examination inclusive critical aspects of examinations. In some cases, exceptions are made on judgment and given in one term like anxiety, pain, and dehydration. Clinical officers should not concentrate most on diagnoses from focused challenge but to risks realized (North American Nursing Diagnosis Association, 2005).

Simply put the difference between acute care and ambulatory care, acute refers to inpatient care while ambulatory refers to outpatient care (Flavin, 2015). An acute setting is a medical facility in which patients remain under constant care. An ambulatory setting might be a non-medical facility like a school or nursing home, but it also includes clinics and medical settings that typically deal with non-emergency issues. As the costs of acute care services increase, patient care is being shifted to the outpatient setting resulting in increased ambulatory care visits. Nurses in an acute care setting are primarily responsible for creating the organization structure of care.
Some questions do not lend themselves to clinical trials or research at all due to ethical limitations. EBP requires a question to be asked clearly, tools to help define a question such as the PICO tool may help (Hoffmann et al., 2013). While anyone can perform EBP, there is a limitation in that it does require training, experience and practice (Hoffman et al. 2013). Due to its’ relatively new arrival EBP training has not been accessible to all health professionals currently working.
There is an opportunity at the Alvin C. York VA Medical Center to improve the safety of patients and staff during the administration of medications on the acute psychiatric units. Currently, all inpatient units at the Alvin C. York VA Medical Center utilize a Pyxis medication station to store patient medications. The current physical location of the medication room and Pyxis stations are not ideal. The Pyxis machine that stores the medications is placed against the back wall close to the nursing station. The location of the Pyxis is next to the medication room where the nursing medication administration cart and scanning system is located.
The physician assistants need to qualify through a general medical examination. They are not required to complete any residency. The examination they need to clear is known as PANCE. Whereas, the nurse practitioners have to qualify through a more specific examination related to some particular medical field, examples of such fields are geriatrics, paediatrics and so on. Physician assistants can prescribe medicines, but the nurse practitioners need to apply at the state level to obtain the permission for prescribing medications.
First, Medicare and Medicaid are similar with what the two insurances covers. But in another way both are different because Medicare had plans while Medicaid does not. Medicaid provides health and medical services for individuals and families with low income. Some of the mandatory services include inpatient and outpatient hospital services, home health, physician services, x-rays, and family planning. A few of the optional services are prescription drugs, dental, hospice, eyeglasses, and occupational therapy.
While Medicare managed plans do not cover hospice costs, many modern-day patients prefer not to stay in the hospital for hospice anyway. By not covering this cost, many patients are encouraged to stay in the comfort of their own home instead. Another limitation of Medicare managed plans is the cap on healthcare spending. In traditional Medicare plans, there is no out-of-pocket limit on healthcare. However, out-of- pocket limits can protect the patient if they need expensive care.
