1. What nursing theory can you apply in planning for the care of Mr. FD? Support why you choose this nursing theory.
I will utilized Orem’s theory which involves 3 central theories: Self-care theory, Self-care deficit theory and theory of Nursing System in planning care for Mr. FD. I chose this theory because of its suitability and practicality in the scenario provided. It is evident that self-care is not maintained by Mr. FD that leads to self-care deficit that requires nursing agency. Orem viewed man as an integrated whole with the capacity of self-knowledge and engagement. Knowledge of self- care requires the fulfilment of self-care requisites which is vital to life and human function.

Environment Environment in Orem’s theory pertains to anything that surrounds the person that might affect his achievement of therapeutic self-care and its behaviour. Orem viewed man and environment as an integrated system. Mr. FD inadequate self-care agency are affected by the environment. Orem’s development of environmental conditions aid in maintaining self-care.
Nursing Nursing in Orem’s theory is a service, an art and a technology.
Nursing is a service of deliberate performed actions by the nurse in helping the client maintain self-care, structural integrity, functionality and development. In Orem’s theory, three variations of nursing system are presented. Wholly compensatory, Partly compensatory and Supportive-educative.
In Mr FD’s case, nursing system approach is partly compensatory. With the difficulty of breathing, pain, persistent cough and weakness Mr. FD need assistance from the nurse in the performance of activities of daily living. The nurse can assist him in ambulation and in basic self-care. He can plan his care with his nurse in order to regain his strength and health. Supportive-educative nursing system will follow. Example: after relieving the difficulty of breathing and pain by providing adequate oxygenation and infections subsided, Mr. FD able to perform task by himself and be educated by his nurse about the use of pain medication, antibiotic therapy, ways to improve oxygenation, proper sanitation, importance of support system and other techniques or methods to maintain his health and meet the therapeutic self-care

Orem’s Self-Care Deficit Nursing Theory One of the most exciting things about nursing is that it is always changing. One of the frustrating things about nursing is that it is always changing. Nursing theory is no different. Theories are continually being updated and revised as worldviews and healthcare itself changes (McEwen & Wills, 2014).
Dr. Jean Watson’s theory of care addressed the nurse to patient ration, according to the method “nursing is positioned with caring of the sick, prevention of sickness, restoration of health and promotion of health. It is the person and their physical, emotional, and psychological needs that are the basic focus of nursing’s attention. In order to care for a patient, the nurse must incorporate all these needs. For example, providing reassurance with an anxious patient who just finished hip surgery. Care also plays a major part when taking care of a unique patient. Caring influences my personal philosophy because it is the most important aspect of nursing.
Overview The case study was about Mr. Kirby, a seventy-two year old widow male with type 2 diabetes who wife died a couple of months ago, and has been living by himself. He has become dependent, and struggles with his self-caring needs. He had a stroke and it resulted in a left-sided weakness. He fell a couple of months ago and fractured his arm bone, which was repaired and he was discharged home.
The concepts of this theory include function of professional nursing, presenting behavior of the patient, immediate reaction, nursing process discipline as well as improvement (Petiprin, 2016). One of the roles of a nurse is to meet the immediate needs of the patient. Patients typically present to a facility with a requirement that should be identified and met by the nurse. The nurse should be able to identify the problematic situation from the patient to be able to address the need for help. The patient develops their perception of events and circumstances based on their automatic feelings and thoughts which causes the patient to have an immediate response.
Nurses intervention are there to coordinate the rhythm between human and environment and assisting the patient in the change process and toward a better health. 9. Dorothea Orem- Orem’s theory creates self care, known as the practice of activity, in which the patient performs his personal routine and needs independently to maintain health and lifestyle, according to age, developmental state, experiences in life, and cultural background. Orem identifies 5 requisites as known as; Activity of Daily Living; .The maintenance of sufficient intake of food and water.
The physiological needs are met when the nurses provide the patient with the necessary amount of food, water, oxygen (if needed), blankets for warmth and sleep by providing pain or sleeping pills (if needed). The safety need will be met by the security of the hospital to help the patient feel secure and safe in his/her environment. Love and belongingness can be created through intimacy (if possible), affection and friendship. This can also be given to the patient by his/her family members or friends. To ensure that the patient’s self-esteem needs are met, the Health Care worker has to help to improve the patient’s confidence and independence.
Orem affirms the theory of nursing systems defines exactly how the patient 's self-care essentials will be resolved or met by the patient or nurse (Self Care Deficit Theory, 2014). Orem classifies three classifications of nursing systems to encounter the self-care conditions of the patient. They are categorized as wholly compensatory system, partly compensatory system, and supportive-educative system (Self Care Deficit Theory, 2014). Nursing systems are a “sequence and structures of measured applied engagements of nurses to protect any disease processes, detect any abnormalities and to bring that patient back to equilibrium (Self Care Deficit Theory, 2014). A good example of this theory would be the nursing process.
Dealing with elimination care, their balance between activities and rest, how they balance solitude and social interactions, the prevention of hazards and the promotion of functioning. (Self-Care Deficit, 2016) The idea of this nursing theory can be applied to solve problems and issues within nursing practice due to nursing theories being the framework and the building blocks of nursing. For the specific theory of Orem’s Self-Care Deficit, the idea of the theory is to be able to assess the patient’s condition, identify the needs and be able to demonstrate communication and interaction with others.
Adams, it is important to address all areas from a holistic standpoint. This would include psychosocial, environmental and medical interventions. While providing care it is important to remember Florence Nightingale’s goal of nursing is to “assist the patient to regain “vital powers” by meeting their needs, which in the end puts the patient in the best condition for nature to act upon.” (Smith & Parker, 2015). Nightingale believed that nurses contribute to restoring health in a direct and indirect way by the management of patient’s environment (Smith & Parker, 2015).
